• Vendors must provide Fort Worth ISD with a completed Conflict of Interest Questionnaire to the address below when they become aware of facts that require it.

• Successful vendor(s) will be notified in writing (manifested by a purchase order or award letter) of the bid/proposal accepted by the Fort Worth ISD Board of Trustees.
• The Fort Worth ISD Board of Trustees reserves the right to reject any and all Bids/Proposals.
